About Your General Health Check
The 15-minute appointment comprises of the following and is conducted by a nurse:
- BGL - Blood glucose level (Finger prick test - non-fasting)
- Height, weight and waist circumference
- BP and Pulse
- Ausdrisk assessment score (Australian diabetic risk screening score)
- General health and diabetes information - the risks and how to lower your risk
- Education on healthy diet and living habits specific to your individual Ausdrisk score

These terms apply to undergoing a General Health Check and you confirm that you have read and agreed to the following terms:
I confirm and agree the following:
- I am at least 18 years of age.
- I give my consent, to have a Genral Health Check, undertaken by a Mobile Screening nurse.
- I understand and agree it is my responsibility to follow up with a medical practitioner regarding any recommendations given to me.
- I give permission for Mobile Screening to retain any information that is collected, including the physical consent form filled out on the day.
- I give permission for Mobile Screening to aggregate my results into a de-identified summary that can be provided to my employer.
Who will see my results?
Only you.
Following the completion of the campaign we provide to your employer a de-identified summary report of how many people were screened and how many (if any) participants needed referrals to their GP.
If you have any questions or concerns regarding the above, please speak to the nurse on the day.
